Effective Practice Nomination Form - Download this document to nominate your district/school as having an effective practice for students with disabilities in literacy, positive behavior intervention, and/or special education instruction.

 Site Visit Protocol - This document functions as a graphic organizer for observation and documentation of the nominated practice during site visits.  The underlying concepts for this observational tool are based on the work of the National Implementation Research Network (NIRN) and on elements of evidence based effective practice in the areas of literacy, positive behavior intervention, and special education instruction.  This document is shared with nominated districts/schools several weeks prior to the visit, and a summary of visitor reactions is shared during debriefing meetings with the district/school staff.

S³TAIR Nomination Validation Online Survey - This is a print version of the online survey which comprises the initial step in validation of nominated practices. After nominees have submitted this survey, they will be contacted by phone to clarify answers as needed, answer their questions, and provide information about the next steps in the process.
